To order by mail or fax, please start by downloading a hard copy order form in either Adobe Acrobat (PDF) format or in Microsoft WORD (DOC) format. Our address and fax number as well as payment instructions are on the order form. |

| After you have completed the form you can either mail it to us along with your check in US funds drawn on a US bank or a money order in US funds. We also accept Master Card, VISA, Discover,and American Express and you can use the printed form above to send us your credit card information if you prefer to pay by credit card but not order online. | |
| If you're ordering by mail you should email us of your intent first. If you've taken advantage of our 30-day free trial, we'll extend your 30-day period by 10 days to allow for mail delivery. This way we try to prevent you having a disruption in your TZO service. | |
